How oral GLP-1s work differently

Oral semaglutide (Rybelsus) uses an absorption enhancer called SNAC (sodium N-[8-(2-hydroxybenzoyl) amino] caprylate) to survive stomach acid and cross the intestinal wall. The bioavailability is roughly 1% compared to injectable, which is why oral doses are much higher in milligrams — 14mg oral vs 2.4mg injectable for comparable effect.

Newer oral formulations — including compounded oral semaglutide, sublingual tablets, and orally dissolving formats — take different approaches to this absorption challenge. Some use different enhancers. Others bypass gastric absorption entirely through sublingual delivery.

Key distinction: "Oral GLP-1" covers several different delivery mechanisms. A sublingual tablet that dissolves under the tongue is pharmacologically quite different from a swallowed capsule. Ask your provider exactly what formulation they prescribe.

Side effects: what changes with the route

Both routes produce GI side effects — nausea, constipation, and appetite changes are common across all GLP-1 formulations. However, oral formulations tend to produce more upper GI symptoms (nausea, gastric discomfort) while injectable formulations are somewhat more associated with injection-site reactions and fatigue.

The dose titration curve also differs. Injectable patients typically start at 0.25mg and titrate up over weeks. Oral titration follows a different schedule, and some patients find the transition between dose steps rougher with oral formulations.

Cost comparison

Oral GLP-1s are not necessarily cheaper than injectables — and in some cases, they cost more. Brand-name Rybelsus carries a list price comparable to Ozempic. Compounded oral formulations vary widely depending on the pharmacy and the delivery mechanism.
